The patient was administered a battery of neuropsychochological testing by the QCM technician who guided, administered and prepared a report for my review. A total of 14 psychological domains were assessed. The time of the report generation was electronically time stamped. The details of which are entered into the medical record. I personally reviewed the report with the patients in great details, going over each domain, the effect of that domain on the patient’s well-being, and areas of improvement that were needed. Based on the results of the report appropriate treatment modifications were instituted. The modifications in the patient treatment plan included medication adjustments, referrals when appropriate and the recommendation of disease management applications. The results of actions take are also listed in the permanent report. After completion of my office visit the QCM tech reviewed the recommendations and insured downloading of the Apps by the patients for use. The actual time using the apps will be collected by QCM and reviewed by me along with changes in the patients domains from the last visit. The progress of the patients well being is chartered to determine the efficacy of the interventions.
